Health screenings (blood pressure, blood sugar, cholesterol)
Flu shots (in the Fall)
Information and Referrals
Training and education
Help with rent and utilities
Clinics, doctors, health insurance
Child safety seats
Fans (in the Summer)
Blue Santa applications (in the Fall)
Help with income tax forms (in the Spring)
To get help, first apply for assistance.
► Call a Neighborhood Center near you to set up an appointment
► Bring the following information:
♦ Photo ID
♦ Proof of address (government ID, or utility bill, or mail)
♦ Proof of income for all adults (18+) in the household for the last 30 days
♦ Social Security Numbers for everyone in the household (if available)
